The referral registry is to establish an online and phone based registry referral service for individual providers and consumers in the home care industry. The development of the registry and the state contributions were assigned to Carina. 0 Form 990, Part VI, Section B, line 11b The Form 990 was prepared by the independent accounting firm SCHOEDEL & SCHOEDEL, Certified Public Accountants, PLLC from information provided by the SEIU Healthcare NW Training Partnership's Finance Director and Carina's Executive Director. Draft copies of Carina's financial statements and Form 990 were first provided to the Finance Director and the executive director of Carina (Carina's ED), who reviewed the Form 990 for accuracy and completeness. Any questions, concerns or issues raised by the Finance Director and Carina's ED were addressed and any necessary revisions were made to the Form 990. The revised Form 990 was then provided to the Board of Directors for its review and approval. Any additional questions, concerns or issues raised by the Board of Directors were addressed and any necessary revisions were made to the Form 990. The final version of the Form 990 was reviewed and approved for filing by the Board of Directors. To ensure that Carina operates in a manner consistent with charitable purposes and does not engage in activities that could jeopardize its tax-exempt status, the Board of Directors undertake periodic reviews which include: whether compensation arrangements and benefits are reasonable, based on competent survey information, and the result of arm's length bargaining; and whether partnerships, joint ventures, and arrangements with management organizations conform to Carina's written policies, are properly recorded, reflect reasonable investment or payments for goods and services, further charitable purposes and do not result in inurement, impermissible private benefit, or an excess benefit transaction. To assist the Board of Directors with its periodic reviews, all proposed relationships and contracts with service providers, as well as all proposed investments that would be made by the organization, are provided to and reviewed by Carina's legal counsel. In determining the executive director's compensation, the Board of Directors uses competent survey information and contemporaneously documents the substantiation of the deliberation and decision.
