The diagnosis is checked by a clinician and case manager. Substance Abuse Funds are for clients who are receiving Substance Abuse treatment. The staff requesting the funds makes sure the clients are eligible. Form 990, Part VI, Section B, line 12c The Board anually reviews all of the agencies policies and procedures. Each Director reads and signs a Conflict of Interest Disclosure. The members are then required to anounce at meetings throughout the year if any transactions might cause a conflict if interest, then recuse themselves during the voting.
