See Schedule O. false false 14564623 Acute Care and Residential Programs-SalusCare is an approved and designated public Baker Act Receiving Facility, accepting individuals who are placed under a 72 hour involuntary hold after they have been deemed a harm to themselves or others due to the direct effects of behavioral health disorder(s). In 2018, 2,112 adults and 990 children were cared for in our crisis units. SalusCare also operates a 29-bed adult inpatient Detoxification Unit. In 2018, the Detoxification Unit served 1,387 adults. SalusCare has Residential care as a part of its treatment continuum for both children and adults. The Vince Smith Center is a juvenile residential treatment facility for adolescents with the disease of addiction. In 2018, the Vince Smith Center served 55 adolescents. Also operated by SalusCare is the Transitional Living Campus, a combination of transitional living/halfway house and permanent supportive housing. In 2018, 411 individuals were served by this program. 6734174 Outpatient Programs-SalusCare offers four outpatient locations in Lee County (two in Ft. Myers, one in Lehigh Acres and one in Cape Coral). In an effort to lessen barriers to care in obtaining outpatient services, SalusCare began offering telehealth services, called Care Anywhere. The organization provided outpatient mental health and substance abuse services in 2018 to more than 9,905 adults and 2,452 children through therapy and/or psychiatric services. Outpatient services is one of the primary services provided by SalusCare with nearly 75% of patients utilizing this service. Patients receive a multitude of services in outpatient programs including screening, individual and group therapy and medical services. Service delivery is dictated and dependent upon the unique and diverse needs of each patient and family. 4501818 Community-based programs-SalusCare offers several community-based programs thus allowing patients and families to be served in their homes or at other locations within the community. Most notably, the Community Action Team (CAT) which provides comprehensive, community-based services to children up to age 21 with a mental health diagnosis or co-occurring substance abuse diagnosis who are at risk for an out-of-home placement, poor academic outcomes or other serious factors. In 2018, 76 families were served in this program. Therapeutic Behavioral On-Site (TBOS) services are also available in the community, including case management services, parenting and evidence-based prevention programs. In this program, SalusCare served 179 patients in 2018. General explanation attachment Form 990, Part III, Line 1, Description of Mission:For the first time as a merged organization, SalusCare earned its three-year accreditation from CARF, the Commission on Accreditation of Rehabilitation Facilities. The following programs received accreditation for both children and adults: residential treatment, day treatment, outpatient treatment, crisis intervention, crisis stabilization and detoxificaion services. Additionally, SalusCare achieved the gold status from Guidestar USA, the premier source of non-profit information. Guidestar USA is a public charity that collects, organizes and presents information on 1.8 million IRS-recognized non-profits. The gold status is the highest level awarded and demonstrates that SalusCare commits itself to the highest level of transparency. Form 990, Part VI, Section A, Line 1:The Executive Committee consists of the Board Chair, Vice Chair, Treasurer, Secretary, and other Board Members as it deems appropriate.
