Form 990, Part VI, Section B, Line 12c At the time of nomination and acceptance of a new board member, all potential conflicts of interest are disclosed as required by policy and by-laws. It is the responsibility of the board member to disclose any conflict of interest that may arise. The EVP of Business/Operations and Director of Accounting monitor all transactions involving any board member, officer, or family member. Any potential conflict of interest is reviewed by the Finance Committee and brought to the attention of the Board as needed. The vote of the Board is documented in the meeting minutes. A new 10 year contract was issued in 2011 and revised in 2016.
