<Desc>CLINICAL POCKET GUIDE FOR NON-RHEUMATOLOGISTS - AXIAL SPONDYLOARTHRITIS (AXSPA) CAN IMPACT EVERY CORNER OF LIFE. THIS POTENTIALLY DEBILITATING BUT MANAGEABLE RHEUMATIC CONDITION PRESENTS REAL CHALLENGES FOR PATIENTS AND CARE PROVIDERS ALIKE, BUT THE RIGHT CARE PROFESSIONAL, INCLUDING THOSE IN THE PRIMARY CARE SETTING, CAN MAKE ALL THE DIFFERENCE. AXSPA IS OFTEN DIFFICULT TO DIAGNOSE PARTLY BECAUSE ITS MOST COMMON SYMPTOM, BACK PAIN, IS A PERVASIVE PATIENT COMPLAINT WITH A VARIETY OF UNDERLYING CAUSES. THIS PARTIALLY EXPLAINS WHY IT CAN TAKE UP TO 10 YEARS TO OBTAIN THE CORRECT DIAGNOSIS. AXSPA AFFECTS ROUGHLY 3.2 MILLION ADULTS IN THE U.S. ALONE, MAKING IT MORE COMMON THAN RHEUMATOID ARTHRITIS. DESPITE THIS REALITY, AXSPA IS NOT ALWAYS WELL UNDERSTOOD BY THE GENERAL PUBLIC OR EVEN MEDICAL PROFESSIONALS. THIS DOCUMENT PROVIDES AN OVERVIEW OF AXSPA, AS WELL AS REFERRAL STRATEGIES AND TREATMENT RECOMMENDATIONS FOR PRIMARY CARE PHYSICIANS AND OTHER HEALTH PROFESSIONALS.DELAY TO DIAGNOSIS - SAA CONTINUES TO COLLABORATE WITH ASIF (AXIAL SPONDYLOARTHRITIS INTERNATIONAL FEDERATION) INITIATIVE TO UNDERSTAND THE DELAY TO DIAGNOSIS ON A GLOBAL LEVEL. INFORMATION GLEANED FROM "HACKATHON" PROGRAMS WILL BE SHARED WITH SPA COMMUNITY THROUGH THE SUMMER 2021 LAUNCH OF A GLOBAL INITIATIVE THAT INCLUDES INFORMATION ON THE WEBSITE AND SHARED WITH NUMEROUS MEMBER ORGANIZATIONS.RHEUM 2 DX AND SPONDYLOARTHRITIS RESEARCHRHEUM 2 DX. PHASE TWO OF A THREE YEAR GRANT. THE GOAL OF "RHEUM 2 DIAGNOSE" IS TO ADDRESS REAL WORLD BARRIERS TO AN ACCURATE AND EXPEDIENT DIAGNOSIS OF NR-AXSPA IN THE RHEUMATOLOGY SETTING. THIS ONLINE SERIES WILL HAVE TWO PARTS; A TOTAL OF THREE EDUCATIONAL PRESENTATIONS GEARED TO RHEUMATOLOGY FELLOWS, AND A TOTAL OF THREE EDUCATIONAL PRESENTATIONS GEARED TO MEDICAL STUDENTS. WORKING WITH SUBJECT MATTER EXPERTS (SMEFS) WHO WILL BE RESPONSIBLE FOR PROVIDING THE EDUCATIONAL CONTENT TO SAA IN A CLEAR AND DIGESTIBLE FORM. TOGETHER WE WILL WORK WITH DVI (VIDEO PRODUCTION COMPANY) TO PROFESSIONALLY (AND VIRTUALLY) RECORD THEIR PRESENTATIONS.EARLY CAREER INVESTIGATOR AWARDS (ECI) - ONE ECI AWARD WAS PRESENTED IN FY 2021 FOR $20,000, AWARDED TO TEJPAL GILL, PHD, ASSOCIATE PROFESSOR AT THE DIVISION ARTHRITIS AND RHEUMATIC DISEASES, OREGON HEALTH AND SCIENCE UNIVERSITY (OHSU), PORTLAND OR. DR. GILL'S MAIN FOCUS IS ON DECIPHERING THE HOST-MICROBE INTERACTIONS LEADING TO THE PATHOGENESIS (ORIGIN AND DEVELOPMENT) OF SPONDYLOARTHRITIS (SPA), ESPECIALLY ANKYLOSING SPONDYLITIS. DR. GILL HAS DEMONSTRATED THAT HLAB-27 ASSOCIATED MICROBIAL DYSBIOSIS IS DISTINCT BETWEEN DIFFERENT HOST BACKGROUNDS AND/OR ENVIRONMENT. AND YET, IN THE SUSCEPTIBLE GENETIC BACKGROUNDS, THE SAME IMMUNE/INFLAMMATORY RESPONSE IS OBSERVED. THESE RESULTS ARE IN CONTRAST TO THE EXISTING NOTION THAT A SINGLE OR A SMALL NUMBER OF MICROBES MAY DRIVE SPA DISEASE. DR. GILL'S CAREER GOAL IS TO INVESTIGATE THE MOLECULAR AND MICROBIAL MECHANISMS LEADING TO THE DEVELOPMENT OF SPA. CURRENTLY, HER GROUP IS INVESTIGATING THE RELATIONSHIP BETWEEN THE GUT MICROBES AND IMMUNE RESPONSE IN HLA-B27 POSITIVE PATIENTS WITH ANKYLOSING SPONDYLITIS. EFFORTS ARE UNDERWAY TO IDENTIFY GENES IN ADDITION TO HLA-B27 THAT MAY CONTRIBUTE TO DEVELOPMENT OF SPA. SINCE MANY ANKYLOSING SPONDYLITIS PATIENTS PRESENT SUBCLINICAL GUT INFLAMMATION, SHE IS WORKING TOWARDS THE DEVELOPMENT OF INTESTINAL ORGANOIDS-A NOVEL TOOL IN CELL BIOLOGY STUDY-TO DECIPHER THE MOLECULAR MECHANISMS LEADING TO HLA-B27-ASSOCIATED LOSS OF GUT EPITHELIAL BARRIER FUNCTION.IN ADDITION TO FUNDING ECI AWARDS, THE ORGANIZATION RECEIVES FUNDING REQUESTS FROM OUTSIDE SOURCES TO SUPPORT RESEARCH RELATED TO SPONDYLOARTHRITIS. BECAUSE THE ORGANIZATION HAS LIMITED RESOURCES FOR RESEARCH FUNDING, IT IS IMPERATIVE THAT ANY RESEARCH PROPOSALS MAKE OPTIMAL USE OF THESE LIMITED RESOURCES. TWO FUNDING REQUESTS WERE APPROVED STARTING IN 2019 AND RUNNING THROUGH 2021. BOTH GRANTS WENT TO CHILDREN'S HOSPITAL OF PHILADELPHIA FOR TWO DIFFERENT PROJECTS. -PAMELA WEISS, MD, MSCE, DIRECTOR OF CLINICAL RESEARCH AND ATTENDING, DIVISION OF RHEUMATOLOGY, CHILDREN'S HOSPITAL OF PHILADELPHIA ASSOCIATE PROFESSOR OF PEDIATRICS AND EPIDEMIOLOGY, UNIVERSITY OF PENNSYLVANIA PERELMAN SCHOOL OF MEDICINE RECEIVED FUNDING FOR AN "EXERCISE FOR DEVELOPMENT OF PEDIATRIC AXIAL DISEASE CLASSIFICATION CRITERIA", A COMPANION PROJECT TO AN NIH RO1 GRANT WITH DR. WEISS TO FILL THE UNMET NEED FOR A PEDIATRIC CLASSIFICATION CRITERIA. THE SAA GRANT WILL BE USED TO FUND IN-PERSON MEETING OF LEADING EXPERTS IN JUVENILE SPA. DUE TO THE COVID-19 STAY AT HOME TRAVEL RESTRICTIONS, THE MEETING IN GHENT (EUROPE) WAS POSTPONED. A NEW PROPOSED MEETING IS BEING PLANNED. -DR. WEISS'S SECOND GRANT IS ANCILLARY TO A PATIENT-CENTERED OUTCOMES RESEARCH INSTITUTE (PCORI) GRANT TITLED THE ROLE OF IMAGING AND SEROLOGIC BIOMARKERS IN PREDICTING FLARE IN CHILDREN WITH SPONDYLOARTHRITIS. THIS WILL BE A THREE-YEAR FUNDED GRANT BY SAA. PRINCIPAL INVESTIGATOR: PAMELA WEISS, MD MSCE, ASSOCIATE PROFESSOR OF PEDIATRICS AND EPIDEMIOLOGY, UPENN PERELMAN SCHOOL OF MEDICINE CLINICAL RESEARCH DIRECTOR AND ATTENDING PHYSICIAN, DIVISION OF RHEUMATOLOGY, CHOP. THIS TRIAL WILL HOPEFULLY IMPACT THE EVIDENCE BASE TO MAKE DECISIONS AT POINT OF CARE OF OUR KIDS WITH SPA WHO ARE IN SUSTAINED REMISSION AND INTERESTED IN TRIALING OFF TNFI. THIS PROJECT AUGMENTS A PCORI FUNDED STUDY.</Desc>
