Primary data was comprised of provider interviews, key informant survey and a public survey available and distributed to Jackson County, WV residents. A population survey was shared by Camden Clark Medical Center. Provider interviews were conducted with representatives from Jackson General Hospital, Coplin Center, and Jackson County Health Department. These interviews highlighted 1. Ongoing and acute addictions and mental health treatment, 2. A vast number of health conditions related to obesity and poor lifestyle choices 3. Care coordination for patients with chronic illnesses, and 4. Educational resources to help citizens navigate through the health community. The key informant survey included health care providers MDs, Administrators, and Clinic staff, EMS Public Health Providers, Public Health Providers, Social Services Agencies and Business leaders. Individuals who participated in the key informant survey were able to provide an additional perspective on Jackson Countys health issues, health behavior and access to healthcare services. Finally, the public survey supplemented the information provided and reinforced the importance of reoccurring topics discussed in the provider and key informant interviews. Group Jackson General Hospital Line Part V, Section B, Line 11 After a thorough review of the primary and secondary data collected, Jackson General Hospital leadership began to establish what was perceived by the public to be the most important health care issues facing Jackson County, WV. The data reflected three major health issues. These included 1 Addictions 2. Mental Health and 3. Obesity/Unhealthy lifestyles. As a result of these findings, The Health Advisory Committee has prioritized these issues above and is currently in the process of developing and implementing solutions to these critical issues. The Health Advisory Committee also plans to seek community involvement with community leaders, funding agencies and community members to address and combat these issues. Group Jackson General Hospital Line Part V, Section B, Line 13b Jackson General Hospital JGH offers discounted care only to individuals who qualify for free care. JGH offers a Prompt - Pay Discount Program - patients who have no third party coverage uninsured or uninsurable may be eligible for a 50 self-pay discount while patients who have a guarantor balance may be eligible for a 20 discount. To be eligible, for either prompt pay discount, the patient/guarantor must meet certain requirements. Group Jackson General Hospital Line Part V, Section B, Line 24 All patients are charged an amount equal to gross charges regardless of payment method. Once FAP eligibility is confirmed, charge amounts are moved to Charity Care and no longer charged to the patient. 3 Morad Hughes Health Center PO Box 720 122 Pinnell Street Ripley WV 25271 Hospital-based rural health center Jackson General Hospital Clinic - Walmart 200 Academy Drive Ripley WV 25271 Provider-based health clinic Jackson General Care Clinic 509 South Church Street Ripley WV 25271 Urgent Care Clinic Part I Line 3c A sliding scale discount is not applicable to Jackson General Hospital services. Payment received for balances over 100 for uninsured patients are eligible for prompt pay 50 discounts for same date of service. For balances of 100 or more, under-insured patients also have the opporunity upon receipt of first statement of receiving a 30 discount. If patients do not respond to first statement discount offer as an option, they have the opporunity upon receipt of the second statement to be eligible for a 20 discount. Uninsured patients not taking advantage of the same day discount can also apply for the first and second statement discounts. These discounts are reviewed and approved by the financial counselors- no discounts are automatically taken by the patient. Part I Line 7 The cost of charity care, Medicaid at cost and other means tested government programs were estimated using a cost to charge ratio derived from Worksheet 2 of the Schedule H instructions. Part I Line 7 The Bad Debt expense included on Form 990, Part IX, Line 25, Column A, but subtracted for purposes of calculating the percentage in this column is 3,388,781. Part III Line 2 The bad debt expense listed on Part III, line 2 is derived from the financial statements. Part III Line 4 Patient accounts receivable are carried at the original charge less an estimate made for doubtful or uncollectible accounts. The allowance is based upon a review of the outstanding balances aged by financial class. Management uses collection percentages based upon historical collection experience to determine collectability. Management also reviews troubled, aged accounts to determine collection potential. Patient accounts receivable are written off when deemed uncollectible. Recoveries of accounts previously written off are recorded as a reduction to bad debt expense when received. Interest is not charged on patient accounts receivable. In order to claim a receivable as a bad debt, it must be estimated that the debt relates to covered services and is derived from deductible, coinsurance amounts and/or nonconvered services that ABN has been signed by patient and processed by Medicare as patient responsible, reasonable collection efforts were made, that the debt was atually uncollectible when claimed, and that sound business judgement indicates there is no likelihood of future recovery. Part III Line 8 Medicare costs and revenues were obtained from the hospitals Medicare cost report. Part III Line 9b We use the same policy and procedure for all patients, regardless of whether we know they have/had qualified in the past. Any patient that presents for assistance will go through the same application process even if they have been previously approved. This is required in case of any financial changes to assure patient still qualifies for assistance, no change in employment or insurance acquired. Once a patient qualifies for charity care, and service was provided, we issue a financial assistance card effective with first date of service that was performed. This card is valid at our facility, for services up to 6 months. They have to re-apply once additional services are performed after termination date. Once approved and within valid time frames if these patients present for services they are then registered as Charity Care instead of Self Pay. Part VI Line 2 Jackson General Hospital utilized the following strategies to successfully perform their health care needs assessment. These included assembly of a Health Advisory Committee comprised of representative from community leaders, health care providers, and citizens to guide the assessment process, conduct primary data collection through surveys of providers, key informants, and the general population, collect and analyze secondary data sources and create a comprehensive community health care services inventory. In addition to this, hospital leadership also outlined specific goals of the needs assessment. These were to identify the following health care issues in the community, vulnerable populations, disparities in the health care services delivery system, health care resources in the community and the next steps to take to meet the health care needs of the residents of Jackson County, WV. Part VI Line 3 Jackson General Hospital employs two financial analysts to meet with patients to discuss financial eligibility for our charity care program. Jackson General Hospital has the qualification requirements, as well as, applications for charity care available at all registration areas. Patients that do not qualify for state or federally funded programs can apply for the charity care program at Jackson General Hospital. Part VI Line 4 Jackson General Hospital JGH is a not for profit, critical access hospital located in Jackson County, WV in the town of Ripley. JGH provides emergency care, imaging services, nuclear medicine, gynecology, ophthalmology, orthopedics, pain management, pediatric, primary and surgical care. JGH also provides a variety of rehabilitation services such as infusion, occupational, physical, respiratory therapy. According to 2019 estimates, Jackson County has a total population of 28,860 and those residents have an average household income of 58,385. Jackson Countys unemployment rate was 5 in 2019. Part VI Line 5 Jackson General Hospitals board of directors is a community board, with all board members living in or around Jackson County, WV. Jackson General Hospital extends privileges to all qualified medical staff in Jackson County and surrounding communities to meet its healthcare needs. Part VI Line 6 Jackson General Hospital is a part of the West Virginia University Health System WVUHS. WVUHS is a not-for-profit corporation formed to serve as part of an integrated health science and healthcare delivery team. WVUHS serves as the parent corporation to an affiliated group of healthcare providing entities. The strategic plan of WVUHS states intent to build a regional health care delivery system in its services area, while offering a variety of options for providers who want to participate. The System maintains a demonstrated commitment to assist rural communities in preserving and improving the health care available to the patients it serves. Part VI Line 6 WVUHSs management is focused on recruitment of staff and employees to meet the growing needs of the aging population in the Systems service areas. Other hospitals in the System include West Virginia University Hospitals in Morgantown, WV, United Hospital Center in Bridgeport, WV, City Hospital, Inc. in Martinsburg, WV, Jefferson Memorial Hospital in Ranson, WV, Camden-Clark Medical Center in Parkersburg, WV, Potomac Valley Hospital in Keyser, WV, St. Josephs Hospital of Buckhannon, Inc. in Buckhannon, WV, and Reynolds Memorial Hospital in Glen Dale, WV. Braxton County Memorial Hospital in Gassaway, WV, Summersville Regional Medical Center in Summersville, WV and Jackson General Hospital in Ripley, WV joined the System in 2019. Part VI Line 6 WVUHS also includes the physician practices of United Physicians Care, Inc. and Camden-Clark Physician Corp which operate in conjunction with the System hospitals along with United Summit Center, a behavioral health center located in Clarksburg, WV. In addition to these practices, the System also includes WVUH-East, Inc. and Camden-Clark Health Services which operate as management companies for their respective hospitals.
