These services include a state-of-the-art Outpatient Diagnostic Facility that opened in 2006. Although the population of the County has been decreasing, the demand for the Hospitals services remain due to the downsizing and service cuts at nearby facilities along with the growth in both the regions uninsured and elderly populations. The Uniontown Hospital offers lines of services that are unavailable elsewhere in Fayette County. Strategies to be implemented FY 2020 - 2022 A Partner with other community agencies and programs which promote healthly living activities. Year one progress - Uniontown Hospital participates on Fayette Countys Healthy Lifestyles Task Force which promotes healthy lifestyles through engagement in Chamber of Commerce initiatives. B Continue to partner with community agencies regarding cardiac rehab/maintenance and other related programs. Year one progress - Uniontown Hospital continues to partner with the Uniontown YMCA to maintain a cardiac rehab/maintenance program C Refine the end of life/palliative care program. Year one progress - Due to the COVID-19 pandemic, priorities have shifted howerver, this strategy is still in progress. D Through the Uniontown Hospital Special Events Engagement Team and the Community Relations Department participate in community events that promote healthy behaviors including area health fairs targeting seniors, students and veterans. Goal Work to improve health outcomes related to substance abuse and mental health. Strategies to be implemented FY 2020 - 2022 A Participate in the Opioid Use Disorder quality initiative. Year one progress - Our case management department participates in the Opioid Use Disorder quality initiative by documenting and submitting information to the state relating to each diagnosis code of opiod use disorder. B Develop better processess to link inpatient or outpatients to providers. Year one progress - We have developed a relationship with Wellness Recovery LLC in Uniontown, PA. Due to the addition of a new provider at their organization, we are currently referring patients to their organization for outpatient services. C Train social workers in American Society of Addiction Medicine. Year on progress - Due to the COVID-19 pandemic, priorities have shifted however, this strategy is still in progress. D Develop a tele-psychiatry program. Year one progress - This goal is still in progress. E Foster stronger relationship with local inpatient psychiatric hospital. Year one progress - We have developed a relationship with Chestnut Ridge Center in Morgantown, WV and are currently referring patients to their organization for inpatient services. Group Uniontown Hospital Line Part V, Section B, Line 11 Priority 3 Health Education. Goal Improve community knowledge related to health care availabiltiy. Strategies to be implemented FY2020 - 2022 A Revamp the Community Physician Referral Line and advertise the Line providing a Hotline for patients to refer to when looking for a new physician. Year one progress - As patients enter our facility, we have a process in place to link those who currently have no primary care physician to a community physician to ensure continuity of care. B Develop communication/marketing tools for patients directing them to alternative care providers related to services that were scaled back at the Uniontown Hospital such as Obstetrics. Year one progress - At the time of the shuttering of some services, we provided flyers both at the hospital and at community provider offices to help educate patients regarding other options for their care. We also messaged these items with staff to speak to with patients and cascaded the options to departments to share as needed. Additional information was also shared via press release and on our social media platforms. C Develop communication related to available transportation to alternative care providers. Year one progress - Due to the COVID-19 pandemic, priorities have shifted however, this strategy is still in progress. Group Uniontown Hospital Line Part V, Section B, Line 11 Priority 4 Preventable Hospital Stays. Goal Improve statistics related to hospital readmissions. Strategies to be implemented FY2020 - 2022 A Develop strategies to help skilled facilities reduce readmissions to the hospital. Year one progress - Due to the COVID-19 pandemic, priorities have shifted however, this strategy is still in progress. Group Uniontown Hospital Line Part V, Section B, Line 11 Some issues identified through the Community Health Needs Assessment have not been addressed in this plan. In initial discussion and subsequent prioritization, Uniontown Hospital considered the levels to which some needs were already being addressed in the service area. Group Uniontown Hospital Line Part V, Section B, Line 13h Eligibility criteria will be based on whether the patient is uninsured or underinsured. For uninsured, liquid assets may be considered for accounts over 5,000. For underinsured, liquid assets may be considered for personal account balances over 5,000. Group Uniontown Hospital Part V, Section B, Line 16i, There are no groups with limited english proficiency that rise to the threshould required under IRC Section 501R. However, language translations of the financial assistance policy and application are available by request at the financial counseling department of the hospital. Group Uniontown Hospital Line Part V, Section B, Line 16j Notice of financial assistance programs are included in the Uniontown Hospital Service Directory and in the Patient Information Books in each inpatient room. There is a paragraph on the second patient statement discussing free or discounted services and directing patients to the proper location to see the FAP. Brochures are available in the emergency waiting room, Patient Financial Services and admissions office which discuss the FAP. 1 Uniontown Hospital Outpatient Diagnostic Center 150 Wayland Smith Drive Suite B Uniontown PA 15401 Outpatient Center Part I Line 3c Eligibility for the Uniontown Hospital Community Charity Care Program will be considered for those individuals who are uninsured, underinsured, ineligible for any government health care benefit program, and who are unable to pay for their care, based upon a determination of financial need. In addition to FPG, consideration of other resources available, such as assets and other financial obligations may be reviewed. For uninsured Liquid assets may be considered for accounts over 5,000. For underinsured Liquid assets may be considered for personal account balances over 5,000. Eligibility exceptions will exist for those patients that are eligible but failed to enroll for subsidized marketplace health plans during open enrollment periods or apply for Medicaid. Other exceptions may exist for certain other situations such as change in patients eligibility level, need of patients who have exhausted their insurance benefits, face extraordinary medical costs, and exceed financial eligibility criteria. Part I Line 6a The last CHNA which breifly describes the primary health care services provided by the Uniontown Hospital was prepared in tax year 2018. That CHNA report is posted on the organizations website and a paper copy is available for public inspection upon request and without charge. Part I Line 7 Uniontown Hospital used the best available data to calculate the cost of charity care reported in line 7. For certain categories, the best available data was derived from a cost accounting system. In other categories, specific cost-to-charge ratios from worksheet 2 were calculated and applied to that category. Part I Line 7 Column f - The amount of expenditures used to calculate column f ties to the total expenditures shown on the Form 990, Part IX Statement of Functional Expenses, Line 25 A. Part II Line 6 - Hospital employees provide representation to various community task forces such as County CVC Sexual Assault/Human Trafficking Task Force and Community Opiod Task Force. Part II Line 8 - The Hospital General Laboratory department participated in a school-based program on health care careers by giving presentations to local high school seniors on careers within the Laboratory. Part III Line 2 Bad Debt Expense During 2019, the Hospital adopted the revenue recognition standard ASU 2014-09. Under ASU 2014-09, the estimated amounts due from patients for which the Health System does not expect to be entitled or collect from the patients are considered implicit price concessions and excluded from the Health Systems estimation of the transaction price or revenue recorded. Bad debt expense was not significant to the audited financial statements for the year ended June 30, 2020. However, the Hospital internally tracks bad debt expense consistent with historical practices and that amount has been reported on Schedule H, Part III, Section A, Line 2. Part III Line 3 Line 3 was estimated by reviewing data from the US Census Bureau to determine the percentage of the population in the Hospitas service area that falls below federal poverty guidelines. This percentage was then adjusted by subtracting the Hopitals 3 year average percentage of individuals eligible under the organizations financial assistance policy. The resulting percentage was applied to line 2 to estimate line 3. Rationale for including bad debt amounts in community benefit It is the Hospitals contention that the cost of bad debt should be stated in Part 1 Line 7 of Schedule H as they represent the costs of provision of services to patients for which the entity has exhausted all recourse for reimbursement. The services provided to patients who present themselves are provided regardless of a patients ability to pay as in line with the organizations charitable mission and service to our community. Part III Line 4 Bad Debt Expense Footnote The audited financial statements do not contain a footnote that describes bad debt expense. They do, however, contain a footnote that describes patient accounts receivable. That note can be found on page 10 of the attached audited financial statements. Part III Line 8 Serving patients with government health benefits, such as Medicare, is a component of the community benefit standard that tax-exempt hospitals are held to. This implies that serving Medicare patients is a community benefit and that the hospital operates to promote the health of the community. The Medicare cost report was utilized to determine the amount reported on line 6. Part III Line 9b The Uniontown Hospitals Billing and Collection policies were approved by the Board of Directors and state that for patients who qualify for charity or are cooperating in good faith to resolve their hospital bills according to our payment options, Uniontown Hospital will not send unpaid bills to outside collection agencies. Part VI Line 2 The Fayette Regional Health System leadership team develops an operating plan every year. This operating plan establishes several priorities for the Uniontown Hospital, all of which are focused on meeting the ever changing healthcare needs of the community. The leadership team develops this operating plan with knowledge gathered from direct contact from patients patient satisfaction surveys outside consultants professional health care groups such as the Hospital Association of Pennsylvania and the Healthcare Financial Management Association and research obtained from various sources including the federal and state government and various other outreach programs and sources. Part VI Line 3 As part of the Revenue Cycle policy entitled Service Assess it states under Patient Advocacy that the health care system will actively assist all individuals in seeking resolution to their financial obligations. This includes helping them understand what services are covered by their health insurance, providing assistance in obtaining health insurance and helping seek eligibility for financial assistance. Currently this program is called the Uniontown Hospital Community Charity Care UHC3. UHC3 is available to all residents of Fayette County and patients will be assisted by Financial Care Consultants that are part of the Patient Financial Services department. The Hospital has brochures located in its waiting rooms referencing all financial services and patient obligations. In addition, the Patient Access staff makes the brochures readily availiable to the population at the time of registration. Part VI Line 4 The Uniontown Hospital is a 145 bed community hospital which is located 50 miles southeast of Pittsburgh in the City of Uniontown, Fayette County. Since 1904 the Hospital has been serving the greater Uniontown and Fayette County area, providing quality healthcare regardless of a persons ability to pay. The approximate population of Fayette County is 134,000. The current unemployment rate is 8.1. Approximately 19 of the population is over 65 with 18.6 of the population below the poverty level and 25.8 of the population eligible for Medicaid. Source US Census Bureau, American Community Survey. 2012-16. Part VI Line 5 The general health and well being of the community residents is central to the mission of Uniontown Hospital. The Hospital responds to the healthcare needs of the community with a number of community education and outreach programs. Additionally, numerous specialized educational seminars, classes and healthfairs are offered free of charge to the public. The Hospital sponsors Blood Drives with the American Red Cross and outreach activities with organizations such as the American Heart Association and March of Dimes. Also, in June 2004 the Hospital joined forces with health organizations and other community groups to organize the Fayette County Community Health Improvement Partnership CHIP. CHIP has initiated numerous programs to benefit the health of the public such as a student immunizaiton program, a program to reduce teen pregnancy and smoking, and several health issue related task forces. Under the auspice of CHIP is Fayette County STEPS initiative which is a national pilot program funded by the Centers for Disease Control which targets chronic disease prevention. The STEPS program builds partnerships among area school districts, workplaces, doctor offices, and neighborhood coalitions to educate and promote healthier lifestyles. Part VI Line 6 Fayette Regional Health System FRHS is a registered 501c3 Pennsylvania nonprofit charitable organization that serves as the parent company for Uniontown Hospital. The Hospital is the only full service hospital in Fayette County. The Hospital includes the following services 24/7 Emergency Department Cardiac Care Center Intensive Care Unit Inpatient Acute Care Orthopedics Infusion Center Clinical Decision Unit Observation Inpatient Rehabilitation Unit Wound Care Center Diagnostic Imaging Surgery Laboratory and Respiratory Care. FRHS also served as the parent of Fayette Physician Network FPN. FPN services include primary care surgery cardiology neurology otolaryngology intensivists and anesthesia. In addition, FRHS is affiliated with the following organizations through joint venture agreements. FRHS has a 35 interest in Southwest Regional Endoscopy Center SWEC, a joint venture formed in January 2004 between FRHS and Southwestern Gastrointestinal Specialists, PC SWGI. The purpose of the joint venture is to provide endoscopy services to patients in and around Fayette County. Additionally, FRHS has a 50 interest in Fayette Oncology Associates FOA, a joint venture formed in February 2004 between FRHS and UPMC Cancer Centers.
