Kokua Mau's ACP Program strives to have conversations serious illness the norm in our community. Our ACP coordinator conducts community trainings across the community. Because of the pandemic, all session have shifted to Zoom but there there continues to be interest in the topic. Kokua Mau is the coordinator for the national POLST program, an out of hospital medical order that documents patient's medical orders. Kokua Mau's website hosts the program. Kokua Mau does ecuation and outreach for professionals and the public across the state, staffs the POLST Leadership Task Force and serves on national committees.
