This beautiful facility is a continuing care retirement community located in the heart of Temple Terrace in the Tampa Bay Area serving the local aging population. Included in the community is a short term rehabilitation center of 163 beds-skilled nursing units. The focus is on healing and using our care team to assist our residents to get better. We are Medicare certified. We participate in many insurance plans including Aetna, Cigna, Coventry, Humana, Blue Cross Blue Shield, Care Plus, Well Care, Freedom, Optimum and United Health Care. The list of participating plans continues to grow each year. Our patients enjoy spacious physical therapy gyms, fully-equipped kitchen for occupational therapy simulation, updated accommodations and bathrooms, wireless internet access, three chef-inspired meals daily, including the preparation of special diets, open and spacious patient living rooms, private dining rooms for family use, planned social functions and wellness programs, bible studies and chapel services and so much more. During 2021, Concordia supported local charitable foundations and also assisted employees with financial support for food and supplies during challenging times. 4179449 4175194 Our assisted living at Concordia of Florida consists of 144 beds. Our warm and expert staff simplify the admission process and helps make the residents and family experience as easy as possible. Residents receive professional assistance with activities of daily living, such as bathing and personal hygiene, mobility assistance, wound care, medication administration, medical appointment scheduling and transportation, three daily meals, fun and engaging activities, barber/beauty shop, libraries, housekeeping, laundry services, chapel services and bible studies to name a few. We also offer memory care assistance with additional security options. We also serve the community by providing benevolent care to many individuals who exhausted their financial resources during the year. We offer aging in place through offering multiple levels of care to ensure a patient is able to stay in the place they call home as long as possible. With this program, our residents receive additional dedicated care from the comfort of their own unit. The reinvigorated benevolent care program provided over $1,200,000 in charity care and continues to benefit many of our assisted living residents. Our improvements to the facility continued to expand to this year for a total of $164,000 in renovations and improvements. Form 990, Part III The Coronavirus (COVID-19) public health emergency (PHE) posed many challenges, as well as opportunities for the organization through the year. We saw a shift in healthcare. The pandemic has created new ways of thinking and solutions that will help to carry healthcare services into the future. In March 2020, the organization implemented its Emergency Disaster Plan. The Leadership Team across the continuum met routinely to continuously evaluate the situation regarding the PHE and ever-changing State and Federal guidance for the safe care of patients with suspected or known COVID-19 infections while attempting to keep all staff safe and healthy. At times, staffing became a challenge when staff needed to quarantine due to exposure or testing positive for COVID-19. A staffing contingency plan was put into place so that the organization would be able to carry out its mission to care for as many patients as possible in the safest of ways. Throughout the COVID-19 PHE, the organization provided focused education on various related topics. These topics included what COVID-19 is, how to prevent the spread, how to care for patients that are positive for COVID-19, mitigation efforts, Personal Protective Equipment utilization (the organization purchased $300,000 of PPE and related supplies throughout the past year) and other infection control practices. This education was provided through varying platforms such as secure messaging, at virtual staff meetings and through the Learning Management Software, Elsevier. Staff have also been educated on COVID-19 vaccinations and the importance of vaccinations as a tool to help end the PHE. A corporate nursing team was established to assist with education and administration related to the COVID-19 vaccinations. The vaccination was administered to fellow co-workers, patients, residents and members of surrounding communities at no charge.
