After any revisions, the form is Part VI, Q. 11B reviewed again by the Treasuer and President, and then it is sent for Part VI, Q. 11B a second time to the entire Board. Upon everyone's satisfaction, Part VI, Q. 11B the form and all schedules are filed with the IRS.
