<FormAndLineReferenceDesc>990, PART I, LINE 1, SIGNIFICANT ACTIVITIES</FormAndLineReferenceDesc>
<ExplanationTxt>The following awards highlight a few of the many sponsored projects that Health Research, Inc has received funding for during the reporting period: Unexpected Complexity in Bacterial Genomes National Institute of General Medical Sciences (NIH) Research from Dr. Wade's lab and others have shown that the textbook view of bacterial genomes is incorrect. Dr. Wade will leverage his expertise in genetics, genomics, and molecular biology, to continue productive lines of research on four overlapping topics that relate to the major research focus of his group: the unexpected complexity of bacterial genomes. Modeling the Influence of Temperature on the Evolution of Vector-virus Interactions National Institute of Allergy and Infectious Diseases (NIH) Studies funded under this project are proposed to comprehensively assess thermal responses of Culex mosquitoes with and without West Nile virus infection and create more informed transmission models under variable temperatures. Given the strong effects of temperature on viral replication rates, the variability of vector species and population responses to temperature, and the documented role of interactions among viral genotype and environment in governing transmission, studies which aim to model temperature need to consider the evolution of dynamic vector-virus interactions. This project proposes to address this gap in infectious disease modeling by simulating evolution under higher temperatures in the lab and utilize these data to inform novel predictive transmission models. Epidemiology and Laboratory Capacity COVID SUPPLEMENTS Reopening Schools Detection & Mitigation of COVID-19 in Confinement Facilities Centers for Disease Control and Prevention Supplemental funds awarded under the current Epidemiology and Laboratory Capacity (ELC) grant for Prevention and Control of Emerging Infectious Diseases in New York State. The purpose of the supplemental funding is to reopen schools in New York State, provide needed resources to implement screening testing programs in schools aligned with the CDC recommendations for K-12, and ensure schools can remain open and operate safely. Additional supplemental funds awarded under the current Epidemiology and Laboratory Capacity (ELC) grant for Prevention and Control of Emerging Infectious Diseases in New York State. The purpose of this supplemental funding is primarily focused on providing resources to confinement facilities for the detection and mitigation of COVID-19. Antibody-based Spike Identification Panel for Vaccine Manufacturing University of Delaware (National Institute of Standards and Technology) The emergence of SARS-CoV-2 variants of concern (VoC) has muddled the vaccine manufacturing landscape. The first-generation SARS-CoV-2 mRNA, viral vector, and subunit vaccines were designed based on the original Spike sequence from the Wuhan/WA01 isolate identified in the United States in early 2020. The past several months has seen the original WA01 strain replaced by variants carrying point mutations and deletions along the length of the Spike protein, including the receptor-binding domain (RBD). In this proposal, we plan to utilize the existing SARS-CoV-2 specific antibody panels to isolate new mAbs that discriminate among the existing RBD VoCs for the purpose of identify testing in vaccine manufacturing. The same reagents may also have utility in stability and potency assays for lot release of various batches of vaccine products. National Initiative to Address COVID-19 Health Disparities Among Populations at High-Risk and Underserved, Including Racial and Ethnic Minority Populations and Rural Communities Centers of Disease Control and Prevention This project includes a comprehensive set of activities to address COVID-19 Health Disparities Among Populations at High-Risk and Underserved, Including Racial and Ethnic Minority Populations and Rural Communities. NYSDOH will engage existing community partners who serve high-risk and underserved populations, including rural communities to develop a coordinated and holistic approach that builds on culturally, linguistically, and locally tailored strategies to reduce COVID-19 risk and ensure equitable access to COVID-19 related services for populations at higher risk, underserved, and disproportionately affected.</ExplanationTxt>

The AIDS Drug Assistance Program (ADAP) began in 1987 as part of a national program to provide free HIV/AIDS drugs to low-income individuals not covered by Medicaid or adequate third-party insurance. New York State expanded the program to include ambulatory care and home care services, insurance continuation and pre-exposure prophylaxis. The programs are funded through partnerships between the State and federal governments and between the State and the New York City, Long Island and Lower Hudson Ryan White Part A regions. Both federal and state statute/regulation governs the programs. The federal Ryan White HIV/AIDS Treatment Extension Act of 2009 includes statutory authority for AIDS Drug Assistance Programs throughout the nation, and associated appropriations provide federal funding to support the programs. In addition, State regulation in NYCRR, Title 10, Sub-part 43-2 governs the application and eligibility determination process and establishes the rights and responsibilities of applicants, participants, and providers. The Programs employ a dual approach to carry out their mission. First, the Programs empower the individual to seek and access care by allowing the individual to choose a provider and receive care/drugs without cost. Second, the Programs supply a stable and timely funding stream to health care providers, enabling them to use the revenues to develop program capacity to meet the needs of uninsured and underinsured people. The Programs are a key component in New York State's response to the HIV/AIDS epidemic. The Programs provide rapid access to antiretroviral (ARV) therapy and ensure universal access to care that is essential to improve health outcomes, achieve viral suppression, and reduce the risk of transmission. The Programs are comprised of five components: ADAP (AIDS Drug Assistance Program) provides free medications for the treatment of HIV/AIDS and opportunistic infections. ADAP Plus provides free primary care services at selected clinics, hospital outpatient departments, office-based physicians, and laboratory vendors. HIV Home Care Program provides coverage for home care services to chronically medically dependent individuals as ordered by their physician. ADAP Plus Insurance Continuation (APIC) can pay health insurance premiums for ADAP-eligible clients. PrEP-AP to prevent HIV infection by providing access to basic primary care services to support HIV-negative individuals at risk of acquiring HIV. The Programs can serve as a transition to Medicaid by providing interim assistance to persons eligible for but not yet enrolled in Medicaid or assist in meeting spend-down requirements. Individuals with health insurance who need assistance with meeting their deductibles or co-payments are also eligible. The Programs will coordinate benefits with their insurance company. Ryan White HIV/AIDS Program (RWHAP) legislation defers to individual states to define what qualifies as low-income eligibility and any additional program restrictions. New York State has codified its responsibilities under the Department of Health Administrative Rules and Regulations State Aid and Funding subchapter (10 CRR-NY 43-2) most recently updated in 2019. The UCP enrollment process has been revised to facilitate same-day enrollment for all participating providers, as well as applicants who apply on their own. The addition of this expansion decreases the program approval time for participants allowing for expedited ART, in accordance with the AIDS Institute policy on Rapid Initiation of Antiretroviral Therapy, which identifies every new diagnosis as an immediate call to action for every provider, with the goal of rapid initiation of treatment and the gold standard of same-day initiation of treatment. This policy supports the "U=U" message that individuals with a sustained undetectable viral load will not sexually transmit HIV. New York State's Uninsured Care Programs are the most comprehensive in the nation, offering a full scope of services to persons with HIV/AIDS. The UCP serves over 23,000 uninsured and underinsured persons living with HIV/AIDS annually. The Programs serve all populations affected by HIV/AIDS in New York State. The Programs served 23,012 participants during the 2021/2022 fiscal year. Cumulative statewide ADAP enrollment as of 3/1/2022 was 117,383, with 17,897 currently active. ADAP Plus has a cumulative enrollment of 105,444 participants with 16,761 active as of 3/1/2022. The HIV Home Care Program had a cumulative enrollment of 5798 participants with an active caseload of 185. ADAP Plus Insurance Continuation (APIC) had a cumulative enrollment of 18,052 participants as of 3/1/2022 with 4,493 active. The majority of ADAP participants are persons of color: 38.3 percent are Black; 35.2 percent are Hispanic; 22 percent are White; three percent are Asian/Pacific Islander/Native American. The UCP/ADAP has developed a program-specific cascade of care which demonstrates that the programs have been successful in linking uninsured and underinsured persons to continuous care and achieving viral suppression. As a result of these activities, 97% of program participants who were active in the program for at least one year show evidence of care and 94% are virally suppressed.</ExplanationTxt>

<FormAndLineReferenceDesc>990, Part III, Line 4b. PUBLIC HEALTH EMERGENCY AND HOSPITAL PREPAREDNESS</FormAndLineReferenceDesc>
<ExplanationTxt>The Centers for Disease Control and Prevention (CDC) Public Health Emergency Preparedness (PHEP) and the Assistant Secretary for Preparedness and Response (ASPR) Hospital Preparedness Program (HPP) funding supports the efforts of the New York State Department of Health (NYSDOH) Office of Health Emergency Preparedness (OHEP). OHEP continues to work with the State Office of Emergency Management (State OEM) and other State Agencies, Local Health Departments (LHD) and private and public-sector partners to build a solid emergency preparedness and response foundation to respond to any crisis or emergent situation in the State. Based on an assessment of NYSDOH capabilities and gaps, NYSDOH has carefully reviewed its progress in designing an integrated and comprehensive health emergency preparedness structure and has strategically prioritized future efforts. 1. NYSDOH OHEP continues to sustain its fully developed Incident Management System (IMS). Accomplishments from the period 04/01/2021 to 03/31/2022 NYSDOH OHEP with the State OEM conducted an Emergency Operations Center (EOC) and Emergency Support Function (ESF) operations training on November 16, 2021 for thirteen (13) IMS personnel. The training is now maintained as a recorded webinar to provide a resource for additional IMS personnel needed for emergency response. NYSDOH OHEP worked with Health Emergency Preparedness Coalition (HEPC) leadership and a multidisciplinary group of community partners to develop the NYSDOH Burn Surge Annex to the Health Emergency Preparedness and Response Plan (HEPRP). NYSDOH participated in real-world mass care and emergency assistance efforts to support response activities following multiple incident types. NYSDOH supported ESF-6 in emergency repatriation, mass care sheltering activations and Disaster Recovery Center (DRC) staffing. The annual Integrated Preparedness Planning Workshop (IPPW) (formerly the annual Multiyear Training and Exercise Plan (MYTEP)) was conducted virtually on June 9, 2021 to identify NYSDOH and other State agency training and exercise activities to build and sustain capabilities over the next five years. 2. NYSDOH OHEP continues to support and provide updated guidance for distribution and dispensing of Medical Countermeasures (MCM), including vaccines and medications, using local Points of Dispensing (POD) and Clinical Operations (ClinOps). Accomplishments from the period 04/01/2021 to 03/31/2022 All 57 LHDs submitted an After-Action Report and Improvement Plan (AAR/IP) to NYSDOH following an MCM Dispensing Full-Scale Exercise. Clinical guidance updates to safely and rapidly provide MCM during POD were included in all 57 LHD ClinOps Plans. Due to the ongoing COVID pandemic, each LHD was required to develop a focused COVID Response Plan. 100% (57 of 57) LHD plans were submitted. Each plan contained fundamental activities and planning efforts related to case investigation, contact tracing and Isolation/Quarantine (I/Q). All fourteen (14) Cities Readiness Initiative (CRI) counties submitted quarterly Technical Assistance Action Plans (TAAP) to NYSDOH. NYSDOH Informatics implemented 100% of the updates identified by the Countermeasures Data Management System (CDMS) workgroup to enhance user accessibility and functionality of the application, including automatic scheduling of 2nd dose appointments based upon duration and medication. 3. NYSDOH continues to support the Medical Emergency Response Inventory Tracking System (MERITS). MERITS is an electronic inventory management system that supports the Strategic National Stockpile (SNS) and the State's Medical Emergency Response Cache (MERC) warehouse operations. This includes processing orders, receiving, shipping, reporting and maintaining a master inventory of all assets. NYSDOH OHEP continues to sustain medical materiel management and distribution capacity to respond to a public health threat with MCM and/or durable medical equipment to provide for a timely and effective response. Accomplishments from the period 04/01/2021 to 03/31/2022 NYSDOH Informatics and Subject Matter Leads (SML) continued to meet and develop the rewrite of MERITS. MERC staff have coordinated with ASPR and conducted drop shipment sustainments for the ASPR Division of Strategic National Stockpile (DSNS) CHEMPACK Project instead of in person sustainments where needed during the COVID-19 response. MERC inventory was replenished. 4. NYSDOH continues to support informatics infrastructure to provide electronic, secure systems and applications for emergency preparedness, response and recovery. NYSDOH will sustain and improve systems for information exchange. In-place architecture was leveraged and new components were added that link NYSDOH with its emergency preparedness and response partners and promote the exchange of data with stakeholders, bi-directionally, while ensuring appropriate privacy protection. Accomplishments from the period 04/01/2021 to 03/31/2022 The immense amount of incoming and outgoing data from applications integrated with the IMS Dashboard was not anticipated at the start of the COVID-19 response. The unknown duration of the pandemic and extensive use of data management tools during response highlighted the need for additional development of the IMS Dashboard to enhance performance and data load handling and NYSDOH Informatics continued this effort. NYSDOH Informatics ensured accuracy of Geographic Information System (GIS) data presented in emergency response applications and mapping tools for real-time situational awareness during emergencies and exercises. Between July and December 2021, NYSDOH Informatics provided 71 Health Commerce System (HCS) trainings, 30 Health Electronic Response Data System (HERDS) trainings and 40 Integrated Health Alert Network System (IHANS) trainings to State, regional and local partners. Due to extensive COVID-19 messaging, the percentage of emergency risk communication on NYSDOH social media channels increased from 65% to 80%. Due to updated communications directory role assignments, acknowledgement of receipt of communications during health emergency responses increased by 20% across partner types. 5. NYSDOH continues to sustain and build public health laboratory testing capability of the NYSDOH Wadsworth Center (WC), to include testing of clinical, environmental, food and water samples. Laboratory testing continues to be a priority area including coordination and communication efforts and building additional capabilities. Laboratory Response Network (LRN)-Biological (B) goals include rapid testing using LRN protocols and reporting to submitters to provide information for prompt decision making. LRN-B will sustain the ability for contacting the sentinel laboratories in New York State (NYS) through the HCS. LRN-Chemical (C) goals include the adoption of technology to process and maintain the positive identification of the large number of specimens that are handled during surge testing and to use the reference materials that are now supplied by the CDC for method improvement and the full validation of analytical protocols. Accomplishments from the period 04/01/2021 to 03/31/2022 100% of all samples suspicious for agents on the LRN Requalification list were tested within two (2) hours of receipt by LRN-B. Six (6) members of the Biodefense Laboratory were trained and are competent to perform LRN clinical and high-risk environmental sample testing. LRN-B laboratory performed testing on 84 specimens or isolates received for Bacillus anthracis, three (3) for Brucella, one (1) Coxiella, seven (7) Francisella tularensis, two (2) Yersinia pestis, 77 for Clostridium botulinum and four (4) environmental. LRN-B laboratory participated in a pilot study to establish MinION WGS capability and PiMA (Plasmid, Integrations, Mutations and Antibiotic) analysis bioinformatics pipeline. LRN-C maintained the qualified status (100%) for all proficiency testing (S-OPNA; U-OPNA; Cyanide; Nit. Mus.; HNPAA; VOC; Abrine/Ricinine; Tetramine; U-SBMSE; S-SBMSE; Blood Metals; U-TES; U-Hg; CVAA). LRN-C developed a new HRMS screening method for 22 fentanyl analogs and aims to participate in the CDC's "Qualitative synthetic opioids proficiency testing program". Two (2) didactic and hands-on trainings were offered in April and May 2021 in which 62 members of the NYS Troopers were educated in proper collection, packaging and transport of suspect threat letters. WC evaluated the MinION and PiMA usefulness for detecting Bacilus anthracis and genetic markers to predict genetic engineering and antimicrobial resistance in this pathogen.</ExplanationTxt>

<ExplanationTxt>PREPARDNESS CONTD. 6. The NYSDOH continues to sustain and enhance its communicable disease surveillance systems that collect information from multiple data streams daily to ensure a baseline of epidemiologic data for NYS. NYSDOH continues to sustain the capability to analyze and interpret epidemiologic data that is critical to the practice of public health. Accomplishments from the period 04/01/2021 to 03/31/2022 Investigations were initiated for 85.8% of Category I disease reports within 24 hours. 100% of Category I diseases were reviewed and assessed by LHD staff. 92.8% of Category II disease reports had an investigation initiated within three (3) business days. 93.9% of Category III disease reports had an investigation initiated within five (5) business days. 94.5% of Category A disease investigations were completed. 100% of Category A diseases were reviewed and assessed by LHD staff. 93.1% of Category B disease investigations were completed. 93% of Category C disease investigations were completed. 100% of notifiable diseases reported to CDC using disease-specific HL7 messaging. 100% of outbreaks and events detected using advanced analytics to start communicable disease investigations and implementation of appropriate control measures. 100% of notifiable diseases reported through NSSP ESSENCE tool for early detection and rapid assessment of bioterrorism-related events nationwide. Program Period Successes: COVID-19 Response NYSDOH personnel provided critical staffing to numerous offices and functions, including the State OEM ESF-8 (Public Health and Medical) and ESF-6 (Mass Care, Emergency Assistance, Housing and Human Services) desks, drive-through diagnostic swabbing operations, serology test sites, call centers and the state-run Mass Vaccination Sites (MVS). These actions helped facilitate response coordination. In response to the rapidly evolving situation, NYSDOH staff continued to develop and distribute several key documents and protocols, including Frequently Asked Questions (FAQ) sheets and infection control guidance documents and advisories for LHDs and health care partners. NYSDOH staff provided leadership and oversight for the implementation of Nonpharmaceutical Interventions (NPI), I/Q, medical surge, education liaising, Personal Protective Equipment (PPE) shortages, movement monitoring, cleaning, disinfection and clinical updates for vaccine operations at the state-run MVS. NYSDOH continued to generate inventory reports using MERITS to quickly assess stockpiled supply of PPE and durable medical equipment, including ventilators, in the MERC and prioritize and expedite purchases during nationwide supply chain shortages. NYSDOH continued to monitor inventory in the MERC warehouses and replenish as needed to maintain supply for the duration of the response. Demobilization planning has begun in coordination with emergency response partners including Division of Homeland Security and Emergency Services (DHSES), State OEM and the Office of General Services (OGS). All state-run testing sites were demobilized prior to October 1, 2021 along with several MVS, but in response to increased cases NYSDOH reopened two (2) testing sites and two (2) additional MVS locations in November and December 2021 and has supported county-run sites into 2022 as well. NYSDOH Informatics continued to support and maintain HERDS and provided support in maintenance of existing daily surveys and creation of new surveys needed by NYSDOH regulatory programs. There are three (3) unique daily surveys for Hospitals, Nursing Homes (NH) and Adult Care Facilities (ACF) to report on and track facility operating status, staffing levels, bed availability and resource inventory. NYSDOH Informatics expanded the Information Technology (IT) support team to reduce limited resource dependencies and provided technical assistance with systems used for downstream analysis of the data to be uploaded in situational awareness dashboards that are used by NYSDOH Executives. NYSDOH Public Affairs Group (PAG) continued to work with the Governor's Office to provide New Yorkers with comprehensive daily messaging to reduce the impact of COVID-19. This included participation in multiple news briefings, proactively interacting with the media, creating new public service announcements and partnerships, maintaining a COVID-19 website and sharing informational resources with LHDs and health care organizations. This phase of the public information campaign included many new elements to reflect ongoing updates to the COVID-19 response, including the continuing availability of booster doses for those age 18+ and vaccine eligibility for children ages five (5) to eleven (11). As part of this effort, the #NYVaxForKids campaign, which promotes incentives and resources aimed at increasing vaccination levels, continued to expand. A statewide sticker-design contest encouraged children to create promotional images intended to encourage their peers to get vaccinated. The judging panel for the contest was comprised of the NYS "Vaccinate, Educate, Graduate" State University of New York (SUNY) and City University of New York (CUNY) scholarship winners from a previous educational campaign. Additionally, broadcast public service announcements aired extensively throughout NYS and focused on vaccine safety to reach those who may be reluctant to vaccinate their children. With the ramping down of government measures aimed at preventing COVID-19 infections, many new communication issues have arisen. As of February 10, 2022 the statewide indoor masking and vaccine requirement was lifted. On March 2, 2022 masking requirements were relaxed in most school settings. With COVID-19 still circulating, the challenge is to encourage people to continue to wear masks when appropriate to protect themselves and to continue getting vaccinated. Spikes in case counts in several counties show that proper precautions are still necessary and helpful and NYSDOH messaging aspires to maintain the balance needed to help keep the infection rate down. WC hired additional laboratory staff to maintain existing laboratory capacity. NYS Department of Corrections and Community Supervision (DOCCS), NYS regional epidemiology offices, NH and veterans' homes adopted use of remote testing request order entry which has improved efficiency allowing WC's Virology laboratory to provide a more rapid turnaround time. The laboratory continued to provide specimen shippers to partners to enhance biosafety practices.</ExplanationTxt>
