BASED ON THE EVALUATION OF THE ORGANIZATION'S TAX POSITIONS, MANAGEMENT BELIEVES ALL POSITIONS TAKEN WOULD BE UPHELD UNDER AN EXAMINATION. Uniting leaders and efforts: Life on Belay facilitates bringing providers and community leaders together under a collaborative strategy, streamlining access to services for families, filling gaps in care, reducing duplicated efforts, and driving progress on core community-wide outcomes. Form 990, Part VI, Section B, Line 11b The 990 was prepared by the executive team and reviewed in detail by the board treasurer for accuracy. It was then provided to all members of the board for review prior to being filed with the IRS. Form 990, Part VI, Section B, Line 15 The independent members of the Board approves compensation for the President, using comparability data. The approval process is documented in the minutes. The president approves compensation for all other positions, using comparability data. Form 990, Part IX, Line 11g Consulting expenses Life on Belay is a small organization with a 100% volunteer board with a small internal team. During this quarter, we have had positive tests for Covid-19. Managing illness in a small team was the reason we missed the date. On June 10th 2022, we realized that the 990 extension due date had passed. We promptly began working on our 990 and completed it as quickly as possible.
