This data will be used to help with sustainability and possible development of placing MCRT in strategic satellite offices to reduce response times. -Continue to support four of eight Mobile Crisis Response Teams (MCRT) and assist with data collection to align with State Reporting and customizing platform to capture data fields needed to begin to automate reporting on a monthly basis. MCRT had more than doubles their call volumes and served approximately 2,700 residents. -In June 2023 the Dyer Care Center constructure of the 32,000 square foot facility was complete and we received the Use and Occupancy Permit. It is the first "Crisis Now Model" facility in the State of Maryland and plans to begin operation is expected within the next few months. We have also developed several printed materials and update Dyer Care Center website to promote the new facility. We continue to host multi-agency .diversion workgroup meetings to continue to strengthen the Emergency Crisis Continuum in Prince George's County through monthly updates from each agency and resolve identified issues in real time. HSCRC DIABETES CATALYST -Successfully received CDC Umbrella Hub Recognition and enrolled six supplier community-based organization to begin billing for National DPP service to build a sustainable model to continue offering these services once grant funding ends. We continue to work with physician practices to identify patients eligible for National DPP and continue the goals of reducing the onsite of diabetes in the State of Maryland. Began applications for Medicare and Medicaid and credentialing process to provide pathway for claims revenue to support our National DPP suppliers and meet State goals of building a sustainable DPP Umbrella Hub Organization. Began partnership with Notre Dame University of Maryland School of Pharmacy to become a DMST Hub to improve access to diabetes self-management service and build a sustainable model through claim reimbursement to reduce dependency on grant funding.
