Form 990, Part VI, Section B, Line 12c Board members are required to complete a conflict of interest disclosure statement upon their election to the board and update the statement annually. Actions are monitored through internal control procedures involving executive management to identify any conflicts. In addtion, action items for board meetings are monitored by board officers and executive management to identify any possible conflicts of interest. This was given to the Board President for board input. The Board President selected several boards members to prepare their evaluation of the EDs performance and what they determined to be goals for the next year.
