Any Questions that any Director has, with respect to the Form 990, are then directed to the Executive Director of AMPLIFY. The Form 990 is prepared directly from the annual audited financial statements. It is also reviewed by the Finance Committee of the Board prior to its being filed. Form 990, Part VI, Section B, Line 12c AMPLIFY has a written Conflict of Interest Policy. Each year a new policy statement is signed by every officer, director and key employee. This is done usually the first Board of Directors Meeting of each calendar year. Form 990, Part VI, Section B, Line 15 The CEO of AMPLIFY, its Executive Director, is given their annual review by the Personnel Committee of the Board of Directors. Initial and annual increases in compensation and benefits are first approved by the Personnel Committee and then subsequently approved by the entire Board of Directors. The Executive Director of AMPLIFY will make recommendations for initial and annual increases in compensation for all other employees to the Personnel Committee. The Personnel Committee will then approve and make its recommendations to the entire Board of Directors, who then will ratify and/or modify them. The Board of Directors have final approval authority. AMPLIFY's annual tax return, Form 990, is also available for public access through various internet web sites. The Form 990 information is completed directly from the annual audited financial statements that are prepared by an outside independent accounting firm.
