Care managers work with individuals and their families/supporters to develop a care plan that will keep them independent in their home. Care managers worked with over 70 contracted providers to authorize a wide array of in-home services and supports. During the 2022-2023 fiscal year, LifeStream provided over 96,000 hours of care management and authorized over 376,000 hours of in-home services.
